We thank you for visiting our website: Pillars of Care.solutions. We would love to be able to help you. By visiting our website it does not automatically establish a patient relationship with Pillars of Care.

Before starting any new diet or exercise plan always consult a medical provider.

 

Initial Patients are conducted in person at our clinic.  After an in-person relationship has been established, telemedicine may be used for follow up visits when an in-person visit is deemed not necessary.

 

Pillars Of Care is not a telemedicine-only practice.  It is an in-person practice.  This is necessary to make sure the best treatment plan can be established.  Telemedicine is available for established patients

 

Out-of-state patients

Yes we are able to accept out of state patients after an initial in-person visit at our clinic has been conducted.  Also if it is deemed an in person visit is necessary to perform a physical evaluation then an in-person visit at our clinic will be needed.  

 

New Patients

To make sure your visit is as productive as possible it is important to complete the intake and forms questionnaire(s) prior to scheduling an appointment.  Once the intake is submitted and received, we can schedule an appointment either on line or by phone.

 

Your Rights : 

*To obtain a copy(paper or electronic) of your medical records.  Please call the office at 405-227-0705 to request records.  Please allow up to 30 days after your request has been submitted.  A reasonable cost-based fee may apply

*Ask for a correction to your medical records if you think the health information is incorrect

*Request communication to contact you in a specific way (ex. home, office)

*Ask us to limit the information we share.  We will be limited by this request except if the law requires this information of your health requires we share this information (ex with a specialist)

*Ask for a copy of your privacy notice at any time

*If you have chosen someone to act as a medical power of attorney or  legal guardian that person can exercise your rights or choices regarding your health information.  Pillars of Care will make sure we have the correct authority to act on your behalf.

*You can complain if you feel we have violated your rights by contacting your office at 405-227-0705

*You can file a complaint with the U.S Department of Health and Human Services Office for Civil Rights by sending a letter to: 200 Independence Ave. S. W.  Washington D.C. 20201 or by calling 1-877-696-6775

 

Your Choices:

Regarding the sharing of your health information, you have the right and choice to have your information shared with close friends, family or others involved in your care.  

*To share information in a disaster relief situation

We will never sell or market your information

 

Disclosures:

We can use your health information and share with other professionals treating you

*We can use your health information to run our practice, improve your care and contact you when necessary

 

How Else Do We Share Your Information?

*We can share your health information with a coroner, medical examiner or funeral director when an individual dies

*For Workers compensation claims

*For government functions such as military, national security services

*For law enforcement purposes

*Responding to requests regarding legal action

 

Electronic Access

We provide electronic access to your health information via Elation Health Electronic Medical Records PassPort Portal

 

Our Responsibilities

*We are required by law to maintain privacy and security of your information

*We will let you know promptly if a breach has occurred

*Follow the duties and privacy practices described in this notice and give you a copy

* We will not use of share your information other than what is described here unless you tell us we can in writing

 

Changes to the terms of this notice

We can change the terms of this notice and the changes will apply to all the info we have about you.  The new notice will be available upon request in our office and on our website
